STEP 1 — CREATE THE FORM
1. Go to forms.google.com and sign in.

STEP 4 — CONFIGURE QUESTION TYPES
• Short answer: use for name, email, phone
• Paragraph: use for comments, feedback
• Multiple choice / Dropdown: use for single-select options
• Checkboxes: use when multiple answers apply
• Linear scale: use for satisfaction ratings (1–5)
• Date / Time: use for scheduling questions
• File upload: enable if you need documents (sign-in required)

STEP 5 — ADD LOGIC (OPTIONAL)
1. Click the three dots on a question → "Add branching"
2. Send respondents to different sections based on answers
3. Example: If "Have you used our services before?" = No → skip to section 3

STEP 6 — CUSTOMIZE APPEARANCE
1. Click the palette icon in the top right
2. Choose a theme colour or upload a header image
3. Recommended header size: 800 × 200 px

   • Link — copy and share anywhere
   • Embed HTML — add to your website
3. Shorten the URL for cleaner sharing

STEP 10 — MANAGE RESPONSES
1. Go to the "Responses" tab
2. View summary charts or individual answers
3. Click the Sheets icon to export to Google Sheets
4. Set up email notifications for new responses
